29-year-old Andrew Gearns from Model Farm Road, Cork City died at Cork University Hospital on October 7th 2020.

Cork Coroners Court heard that he was rushed to CUH following a suicide attempt at Cork Prison on the 28th of September 2020.

His mother Aideen told the court that Andrew appeared to be hallucinating when he phoned her earlier that day and said he had been slashed in the face while walking in Blackpool and had also gone to Mayfield for his tea.

His mother phoned the prison immediately to tell them about the call and was told that they were aware of the situation and would keep a 'closer eye' on him.

Nurse Anna Lyons told the inquest that Andrew Gearns guaranteed his safety on two occasions during an interview 6 days before that and had denied any thought of self-harm or hurting himself.

During a Covid protocol check on the 27th, Nurse Lyons said he was physically well but told her he had been slashed and stabbed during an outing.

A GP review was scheduled for the following day and a check with a psychiatrist was organised.

Prison authorities at the inquest indicated that he was checked 13 times on the day he was found unresponsive.

The inquest continues tomorrow.
