A final report into the scandal noted a number of shortcomings within the service - giving examples of unempathetic call takers and little evidence of checks by supervisors.
Between 2019 and 2020, over 22,000 emergency 999 calls were inappropriately handled, with more than 3,000 relating to domestic abuse.
Chair of the Policing Authority, Bob Collins, says the report identified specific failings within the service:
"I think it is a source of considerable concern that in listening to the calls, Derek Penman found that there was a lack of empathy, a lack of understanding, sometimes an abruptness or inattention and, apparently critically, a difficulty with requesting, recording and validating information; which information is key to the Garda response."
