Fiosrú, the Office of the Police Ombudsman, is investigating a fatal road crash in Mallow.
It happened at around 6 o'clock yesterday evening at Ballyellis.
Gardaí have confirmed a woman was killed in the collision and the incident has been referred to Fiosrú.
The road remains closed while technical examinations are carried out at the scene, and local diversions are in operation.
